After starting up qgis, I selected Layer > Add Vector Layer from the menu, then checked the radio button for "Database". The expected form fields appeared, but there were no options available to select in the database "Type" drop-down; it was empty. Thus, I couldn't actually make any database connections. I then installed qgis using the instructions at http://www.kyngchaos.com/software/qgis instead, and that copy has the database type dropdown correctly populated with ODBC, ESRI Personal GeoDatabase and so on, so I believe the original problem was related to the macport installation in particular. As an aside, with the macports install I got an error on startup that said: {{{ Unable to load GdalTools plugin. The required "osgeo [python-gdal]" module is missing. Install it and try again. }}} I don't think this is related to the database connection problem, but it's worth noting.
