Files should be attached directly to the ticket, not provided as a link to a third-party site. Do you wish to maintain this port? If so, your obfuscated email address should be in the maintainers line, optionally with "openmaintainer" added to indicate that you allow others to update the port as well. If you don't want to maintain this, I can maintain it; I maintain many of the other php modules. We also need to indicate the license. According to the APCu page on PECL, this is under the PHP license version 3.01, so "PHP-3.01" would be the right value for the license field.
