I noticed that even though the upstream tarball already existed in /opt/local/var/macports/distfiles/db4/4.6.21_5 from when I installed that version, the exact same upstream tarball was downloaded and placed into /opt/local/var/macports/distfiles/db4/4.6.21_6.
