I had cairo @1.12.14_0+x11 and gimp2 @2.8.4_6+help_browser+python27+quartz installed. If I install cairo with +quartz +x11 then it works. It appears that when gtk2 +quartz was built, cairo was installed with +quartz variant but this was later changed (perhaps by installing something that wanted cairo +x11). Your solution is the correct one in any case. Note that problems like this often occur when people try and install both +quartz and +x11 programs at the same time.
