Attached is a launch.plist which makes use of this and launches the mysql server when "/opt/local/var/run/mysql5/mysqld.sock" is accessed. If the port maintainers are interested, I could (try to) create a variant for this configuration. You may want to suggest it to the macports-dev mailing list.
