Doing the suggested forcing is a failure: lots of files get rewritten and afterwards qt3-mac is broken.
MacPorts was not suggesting you should force the activation. The wording of the message could probably be improved. MacPorts was explaining that if you know what you're doing, you can force the activation, with all the consequences that entails. The purpose of forcing an activation is to move aside the files the port would otherwise conflict with, with the consequence that the port that installed those conflicting files might not work anymore.
