i opened http://xquartz.macosforge.org/trac/ticket/636 back in september
and jeremyhu suggested i write here to try and get the issue addressed.
from the ticket:

ever since upgrading from 2.7.2 to 2.7.3, i now see X11.app stop bringing
it's windows to the foreground when switching back to it. this doesn't
happen immediately. if i restart X11.app, things work for a while, but then
eventually break. once broken, the bad behavior is reproducible every time.
i only run one X application (Visual SlickEdit). using the <CMD>-2 keyboard
shortcut will bring the window back to the foreground. sometimes, like
right now, that won't even work and i'll need to minimize the window and
i'll need to use Window -> Bring All to Front to get it to come back up.

The first release candidate for 2.7.6 is available for download.
The main changes in this release are updates to newer releases of various modules, including various protos that are necessary for building the 1.15 server (although it is still shipping with the 1.14 server).
There is one XQuartz-specific change. The display lock file is now correctly deleted on quit, so you should no longer get incrementing display numbers on each launch.
If you're on the beta update feed, you should see an update notification when running XQuartz.app, or you can see the full change log and download the release from http://xquartz.macosforge.org/trac/wiki/X112.7.6
