 One more potential issue and solution:

 chmod g+rx /opt/local/var/db/mysql5/mysql

 It caused the server startup to fail which in turn created the same ERROR
 2002.

 It seems that there were several installation problems leading to this
 error, so if this isn't the correct place, please triage it.
