 I think your #13 is the key.  For some reason, Myth's duplicate checking
 is tripped up.  Could you please check the relevant settings on your
 rules:
 Manage Recordings>Recording Rules then select a rule (say Cat in the Hat)
 and navigate to Schedule Options.  Please confirm that the defaults are
 set:  "Use any available input", "Match duplicates using subtitle &
 description", "Look for duplicates in current and previous recordings".
 There should be no "Schedule Filters" applied.

 Also, please go to:
 Manage Recordings>Previously Recorded then select an entry and press "i"
 (for Info).  In the resulting screen, press "i" again for extended info.
 I presume the Titlle, Subtitle and Description fields look reasonable?
 There should be a Program ID like "EP01311784".
