 I think that this bug depends on a certain sequence of events.  I have had
 it install correctly on a clean install of Macports and I was able to get
 it to fail using the same process that I originally encountered.

 I think it has to do with what version of docbook-xml are already
 installed prior to installing xmlto.  Since xmlto depends on version 4.2
 and there is already a 4.5 ... I think the time it worked, something else
 had already installed a newer version of docbook.

 Sorry that I cannot recall the what port I was installing when it worked.
