Erm, I can't...I'm trying to do this running rootless (not
the same account, but it is the same X11 install on the
same machine) so I fired up the X version of firefox,
got one of its menu dropdowns open, ran xwininfo in
terminal.app and it says

xwininfo: Please select the window about which you
would like information by clicking the
           mouse in that window.
xwininfo: error: Can't grab the mouse.

Maybe that tells you something useful anyway? All the
windows this is happening with are ones tied to the mouse
like that.

I'm not seeing any ghosts with this set up running rootless,
only with the full-screen set up and fvwm2. I went back and
checked it was still happening in the other account, and it
is completely reproducible there (logging out and logging in
as the other user, no fus).

Some more info on the mouse anomaly I noted. When switching
back to X from Aqua and reactivating the fvwm root menu that
just became a ghost, the mouse position has been lost and
the menu appears in the centre of the screen, even though
the mouse click was somewhere else (it should appear under
the mouse) and the ghost was somewhere else too. The ghost
disappears when the real window appears.

And to clarify. The ghost appears first in X11 after going
to Aqua and then coming back. Then another switch to Aqua
shows up the ghost in Aqua too.

P.S., I'm pretty sure I have everything installed in the
right order. I checked my command history and file
timestamps, and X11 info, and anyway, I don't think I could
have managed to run full-screen successfully if I didn't
have all the right bits...I initially tried without
installing 2.3.2-beta1 and I just got a library error.
